Utredning: Vad behövs data från klinisk praxis till? Tillsyn, HTA och nationell styrning av läkemedel och medicintekniska produkter som synvinkel
Ständigt ökande kunskap om hälsa och sjukdomar, ökad förmåga att analysera stora datamängder samt utveckling av precisionsmedicinen skapar ett behov av och möjligheter för användning av data från klinisk praxis (real-world data, RWD).
RWD är data som hänför sig till patienternas hälsotillstånd eller användningen av hälso- och sjukvårdstjänster och som samlas i olika datalager såsom patientdatasystem, register, datasjöar och biobanker. RWD används bland annat i patientarbete, kunskapsbaserad styrning samt forsknings-, utvecklings- och innovationsverksamhet. Synen på olika aktörers databehov och möjligheten att utnyttja RWD håller först på att utvecklas.
Säkerhets- och utvecklingscentret för läkemedelsområdet (Fimea) har på uppdrag av social- och hälsovårdsministeriet utarbetat en utredning om användningen av RWD. Fimea överlämnade sin utredning i dag den 27 november till social- och hälsovårdsministeriets kanslichef Päivi Sillanaukee.
En förutsättning för användningen av RWD är enligt utredningen en högklassig engångsanteckning av uppgifter. Utredningen lyfter fram konkreta nationella och internationella åtgärder genom vilka det är möjligt att främja en ändamålsenlig användning av RWD.
I utredningen beskrivs ändamål för RWD med avseende på tillsynen av läkemedel och medicintekniska produkter, bedömningen av metoderna inom hälso- och sjukvården (HTA) samt den nationella styrningen av läkemedelsbehandlingar. I dessa fall behövs sjukdoms- eller patientgruppsspecifik data om bland annat genomförandet av behandlingen och slutresultaten. Det finns dessutom ett allt större behov att förena genomdata och data om biomarkörer med uppgifterna om en patients hälsa.
Åtgärdsförslagen i utredningen är avsedda för beslutsfattare och sakkunniga som deltar i tillsynen av medicintekniska produkter, bedömningen som gäller användning och styrningen. Förslagen gäller fastställande av databehov, översyn av processerna, stärkande av kunnandet samt kommunikation om databehov och datakrav.
De i utredningen identifierade möjligheter och begränsningar som gäller användning av RWD kan tillämpas på sekundär användning av social- och hälsouppgifter också i större omfattning.
